Automatically send subscription receipts to patients every billing cycle

A subscription receipt will be sent out automatically via email every billing cycle (if the transaction is processed successfully), so that patients can access their receipts without manually requesting from the organization. 

Subscription receipt will include the following information

• Provider’s name
• Patient’s name
• Provider’s specialization (if applicable)
• Provider’s registration/license number (if applicable)
• Logo
• Billing type (monthly or yearly)
• “Paid” status
• Service name
• Custom information configured by the organization

Bugs

• Fixed an issue with characters jumbling around when printing completed forms, so that no matter where the form is downloaded from, selected options will be displayed appropriately
• Fixed an issue with showing the form count to patient users when an internal form is assigned, so that internal forms will be excluded from the form count when being viewed by a patient user
