Viewing your Carelogic medical records via your patient portal

As a patient, you can now view your records by logging into your patient portal on a computer device. Within this section, you can view all or some of the following information, depending on the configurations that your healthcare provider organization has determined.

  • Demographics information stored on your medical records
  • Active medications
  • Active diagnoses
  • Completed lab results 

Demographics information 

Within this tab, you will be able to view the following information

  • Demographics
    • First name
    • Last name
    • Gender identity
    • Date of birth
    • Race
    • Ethnicity
    • Preferred language
  • Addresses
    • Home address
    • Work address
    • Billing address
  • Contacts
    • Email address (not available for Carelogic patients/ clients)
    • Home phone
    • Cell phone (not available for Carelogic patients/ clients)
    • Work phone (not available for Carelogic patients/ clients)

Please note that you will only be able to view the data entered by your healthcare provider in your Patient Portal right now. If you do not see some of your information in this tab, it means that your healthcare provider might not have entered it in the EHR.

Medications 

Within this tab, you will see the list of all active medications the healthcare organization has access to:

  • Medication: the name of the medication
  • Dosage: how the medication should be used
  • Date approved: the date when the prescription was initially written
  • Quantity: the amount to be dispensed for one fill
  • Learn more: the link to another page showing more information about the medication

 

Please note that you will only be able to view the data entered by your healthcare provider and won’t be able to request a medication refill via the Patient Portal at the moment. 

Diagnoses 

Within this tab, you will see the list of all active diagnoses your healthcare provider has prescribed:

  • Diagnosis: the condition/diagnosis that has risen to a level of concern
  • Code: the identification code of the condition/diagnosis
  • Date diagnosed: the date this particular condition/diagnosis was created in the system
  • Learn more: the link to another page showing more information about the diagnosis

Lab Results 

Within this tab, you will see the list of all lab tests and results related to your condition:

  • Test: the name of the test ordered to evaluate your condition
  • Order by: the name of the healthcare provider who ordered the test (not available for Carelogic patients/ clients)
  • Date ordered: the date when your specimen was collected for the test
  • Date received: the date that this version of the test was made available to providers
  • Result: the result of the test
  • Abnormality: Interpretation of normality or degree of abnormality (not available for Carelogic patients/ clients)

Reminder, if you are not viewing any of the tabs listed here, then it is due to your agency's configurations and are not available in your patient portal.
