How To Complete A Form or Assessment

Overview

Your healthcare provider may request that you complete an interactive form (i.e. a new client intake form) or assessment (i.e. a PHQ-9 assessment) prior to, or after, your appointment. If you have been assigned a form, please follow the steps below to complete it on your preferred device.

In addition, please allocate enough time to complete the forms assigned as nothing will be saved unless you click on the "Submit form" button. If you have to use your mobile/tablet device to complete forms, please make sure that you log into your account on a web browser mobile application.

Completing as Assigned Form or Assessment 🖥

You will receive an e-mail indicating that you have been assigned a new form or assessment that needs to be completed. Click "Complete form" to get started

If you have not yet registered your account, please follow the steps to set up your account. Once complete, you'll be able to complete the assigned form.

Some providers give users the option to complete forms later, these forms will have a "Complete form later" button under the submit button. Otherwise, forms are required to be completed prior to the scheduled appointment.

Once a form has been filled in completely, you'll be able to hit the "Submit form" button to complete this process. You will be able to add your signature while completing the form. 

Once submitted, you'll be brought back to your appointment summary. Clicking on the clipboard icon will give you a summary of all completed and outstanding forms for your appointment

Completing a Form or Assessment on a Mobile Device 📱

Navigate to your mailbox app and click on the "New Form to Complete" notification email you received from your provider. Click on the "Complete Form" prompt and follow the instructions to download the mobile app from the App Store or Google Play Store. Proceed to the next steps below once you have the app installed. 

Open the mobile app and log into your account. You will be prompted to complete any forms that have been assigned to you by your provider:

Proceed to fill in the form truthfully, all required questions are marked with an asterisk (*). Once complete, click on "Submit". You'll receive a visual confirmation that you have successfully completed the form
