How to Complete a Form or Assessment

Your healthcare provider may request that you complete an interactive form (i.e. a new client intake form) or assessment (i.e. a PHQ-9 assessment) prior to, or after, your appointment. If you have been assigned a form, please follow the steps below to complete it on your preferred device.

We strongly recommend using computer to access your forms. However, if you're using a mobile device, please click here.

Please allocate enough time to complete forms assigned as nothing will be saved unless you click on "Submit" button. If you have to use your mobile/ tablet device to complete forms, please make sure that you log into your account on a web browser mobile application. 

Tip: If you are having trouble loading or submitting the forms, try completing the form on Google Chrome. Google Chrome is the preferred browser to be used for form completion. You can download Google Chrome here.  


Completing as Assigned Form or Assessment

You will receive an e-mail letting you know that you have been assigned a new form or assessment that needs to be completed.



If you have not yet registered your account, please follow the steps to set up your account. Once complete, you'll be able to complete the assigned form.



If you have already registered your account and are returning to complete a form that you have not yet completed, you will be prompted to submit your form before you're able to access the rest of the platform.



If you want to skip the form and come back to it later, scroll the to bottom of the form and click on "Complete form later" if you want to skip 1 form, or "Complete all optional forms later" if you want to skip multiple forms at once.



Click on the clipboard icon and you will get a list of forms that your provider has uploaded and assigned to you to complete. The number indicates the number of forms that have been assigned.  



Please complete the form/assessment as requested. A question marked with an asterisk (*) indicates that the question is mandatory and must be answered. 



If you are unable to submit the form, make sure that you have completed all of the answers that are required. If you have not fully completed the form, the mandatory fields that you missed will be bolded in red. 



Once the form is complete, your healthcare provider will review your responses and may choose to discuss further during your appointment. 


Completing a Form or Assessment on a Mobile Device

Navigate to your mailbox app and click on the "New Form Assigned" notification email you received from your provider. Click on the "Complete Form" prompt and follow the instructions to download the mobile app from the App Store or Google Play Store. Proceed to the next steps below once you have the app installed. 

Open the mobile app and log into your account. You will be prompted to complete any forms that have been assigned to you by your provider:



Click on "Submit" once you finish all mandatory questions in the form. You can also "Skip" or "Skip all optional" if you need quick access to your appointment. Once you finish all forms assigned to you, a "You are all set for your appointment" screen will pop up to confirm your next step.

Do you need to access Forms because you skipped them?

If you skip your forms in order to access your appointment faster and are looking to complete them after your appointment, please follow the steps below.

Navigate to the "Attachments" section on the bottom navigation bar (indicated by the small paper icon).



Select the form you want to complete from the list of available options. 



Once you complete all questions, click on "Submit" to submit your responses.

Please note:

  • You can only edit incomplete forms. Completed forms will only be available for review
  • Each form will have a "Incomplete" or "Complete" status associated with it 
  • You will need to complete all required questions before submitting a form
