How to send Interactive Forms and Assessments

OnCall Health now offers the ability to assign forms and clinically validated assessments to your clients, where appropriate. Interactive forms can be electronically assigned at regular intervals to complement your in person or online video appointments.

To assign a form, please follow the steps below:

  1. If you haven't already done so, please contact support@oncallhealth.ca to add a new form to your account
  2. After creating your in person or online video appointment, click the paper clip icon. Select "Assign a form" and click "Assign to {{Client Name}}"final_5e594d5abf22c30016c2da39_118851.gif
  3. The clip board icon to the far right of your appointment will allow you to track whether or not a form has been completed. If the circle is blank, the form is incomplete, if the circle is in green with a checkmark, the form is complete


4. Once assigned, your client will receive an e-mail letting them know they have a new form to complete. The client will be prompted to log into their OnCall Health account in order to complete the form.

5. Your client will be prompted to complete the assigned form as soon as they log in. They will also have the option to "Complete later."


6. Once completed, you will be able to review your client's responses. You will receive an email with a link that will take you directly to your client form.
