Your healthcare provider may provide you with files (i.e. terms and agreements) prior to your appointment. If you have been assigned a file, please follow the steps below to download it onto your preferred device.
When your provider shares a file with you, you will receive an e-mail with the subject line "(Your Provider) has shared a file with you".
The email will let you know that a new file has been shared with you and provide a direct link to your account. Click, "View File" to log into your account.
If you have not yet registered your account, please follow the onscreen instructions to do so.
If you already have registered your account, proceed to log into your account. Once you have logged into your account, navigate to the Documents tab in the top menu.
From here, you will be able to access a gallery of all forms and files that were shared with you.
Navigate to the Documents tab and select the Files section and you will be able to see all the files shared by your provider and the date that it was shared with you.
Click on the file you'd like to view and it will be automatically downloaded to your device.
