How to download Interactive Forms & Assessments as a PDF

OnCall Health now offers the ability to export forms as a formatted PDF. This allows you to download the form that your client has completed as a PDF directly to your computer any time for your own documentation, or if you'd like to save the file to print.  

If you currently do not know how to assign a form, follow our guide on how to send interactive forms and assessments. 

Let's do a walkthrough on how to download Interactive Forms and Assessments in the OnCall Health platform as a PDF.

There are 3 ways to access the form that has been submitted by your client

Option 1. Via OnCall "Patient has submitted a response form" email

1. Once a form is completed by a patient/client, you will receive an email notifying you that the form has been completed.

2. Clicking on "Review" will prompt you to the form that was submitted by the client. Please make sure to log into your account in order to be prompted directly to the form. 

3. You can check the status for the form in the top left corner

 

4. Scroll to the end to the form and click "Print".

Please note that clicking on "Export" will automatically download the form as an Excel file.  

5. Clicking "Print" will open up a new prompt with a print preview screen. 

6. We will be saving the file as a PDF. Go to "Destination" and ensure that you select "Save as PDF". 

 

7. Click "Save". You will be prompted to name your file and save it in the appropriate folder on your computer. 

 

8. The patient form will be in the folder you saved it in. You can now access your patient forms as a PDF!

Option 2. Via the appointment 

1. Log into your account.

2. Navigate to the appointment that the form you wish to download was assigned and click on the paper clip icon on the top right corner.

3. You can check the status for the form in the top left corner.

4. Scroll to the end to the form and click "Print".

6. We will be saving the file as a PDF. Go to "Destination" and ensure that you select "Save as PDF". 

 

7. Click "Save". You will be prompted to name your file and save it in the appropriate folder on your computer. 

 

8. The patient form will be in the folder you saved it in. You can now access your patient forms as a PDF!

Option 3. Via "Forms & Files" tab in Roster

1. Log into your OnCall Health account and navigate to Roster tab.

2. Navigate to "Forms & Files" tab within Roster.

3. You will see a gallery of all the forms that were assigned to this Roster contact and download the form by clicking on the form. The checkmark on the right indicating that the form was completed and submitted.

4. You can also check the status for the form in the top left corner when reviewing the form. 

4. Scroll to the end to the form and click "Print".
