How to Assign Forms on Mobile Devices

If you are using your mobile/ tablet device as your main device to connect with patients, you are now able to assign interactive forms to your clients on our mobile app. 

1. Log into your account and navigate to the appointment that you would like to assign form.


2. Click on the Paperclip icon Screen_Shot_2020-09-17_at_12.28.02_PM.png and choose "Assign Form".


3. Choose the desired form in the list of forms available in your clinic.


4. Click on "Assign Form" to assign the form to your desired participant. In the case of a group session, you will have the ability to toggle the blue button to decide which participant to assign the form.
