How to Funnel answers from Forms to Custom Roster Fields

As a provider, you can save time by pushing patient answers from forms directly into their Roster Contact. With the added feature in our Form Builder tool, you can do so by mapping form questions directly to the roster fields, so the answers to these mapped questions will automatically appear in the Roster Contact's Profile page.  

In order to funnel answers from Forms to the client's Roster Profile page, you will need access to:

  • Custom Roster Field tool
  • Form Builder tool

If you do not currently have access to these tools, please contact your Account Manager directly for more information. 

Using the Form Builder tool, you can funnel answers to the following question types:

  • Short answer
  • Single checkbox
  • Step 1: Navigate to Configuration tab in your account

  • Step 2: Navigate to Roster Fields tab & Create the Custom Roster Field

    If you are not familiar with how to create a custom Roster field, please refer to this article for more instructions
  • Step 3: Navigate to Forms tab to create a new form

    If you are not familiar with how to create a new Form using the Form Builder tool, please refer to this article for more instructions 
  • Step 4: Insert the question and map to the Roster field

    If you want to map a short answer question, add a new "Short answer" question type to your form and map the field using the "Map a Roster field" dropdown on the right

    If you want to map a checkbox, add a new "Single checkbox" question type to your form and map the field using the "Map a Roster field" dropdown on the right

Once you finish mapping the questions in a form to your custom Roster fields, when a patient submits their answers to the form, the Roster fields will be automatically updated. If the patient answers the form more than once, the most recent answer will be what appear in the Roster field.
