Overview
As a provider, you can save time by mapping client answers from their assigned forms directly to fields that exist on their Roster Contact. Using Form Builder, you can map answers to the following question types: (1) short answer, (2) single checkbox. In order to funnel answers from Forms to the client's Roster Profile page, you will need access to:

To get started, navigate to the Configuration tab in your account by clicking on your username in the top right corner of the screen. Navigate to the Roster Fields tab and click Create custom field.
If you are not familiar with how to create a custom Roster field, please refer to this article for more instructions.
Once your custom Roster fields are in place, navigate to the Forms tab to create a new form.
If you are not familiar with how to create a new Form using the Form Builder tool, please refer to this article for more instructions.
Insert the question and select the field you'd like to map the answer to using the "Map a roster field..." dropdown list.
Once you finish mapping the questions in a form to your custom Roster fields, when a patient submits their answers to the form, the Roster fields will be automatically updated.
Side Notes
- If the patient answers the form more than once, the most recent answer will be what appear in the Roster field.
- If you are creating a form that will be available on your booking page, you can also map patient answers to Custom Roster Fields. Once patients answer those questions on the booking page form, their answers will be automatically added to the existing fields in Roster. This type of form is perfect to collect demographic information that would not frequently change over time such as date of birth, contact information, emergency contact information, allergies, major medical history, etc
