How to Accept Files in Patient Forms

If you've assigned your clients a form to complete, using the 'File Upload' question type can be a great way to collect documents, files, and/or images.

The following steps will walk you through adding the File Upload block to your custom form configuration. 

📝 Create Form: Start by creating a new form within your account, or edit an existing form previously created to add file upload. Click here to learn how to create a form using Form Builder.

📂 File Upload: After naming your form, click on Insert Content and select the File Upload question type. Please note that you can currently receive the following file types: JPG, PNG, PDF. You will have the ability to add a question or description above the upload button.

⬇️ File Download: You will be able to download the file by clicking on the assigned form from the Appointment Summary card. An email notification will also be sent to you when the form has been completed by the client.
