How to Accept Files in Patient Forms

Overview

If you've assigned your clients a form to complete, using the 'File Upload' question type can be a great way to collect documents, files, and/or images. The following steps will walk you through adding the File Upload block to your custom form configuration.ย

Create Form ๐Ÿ“

Start by creating a new form within your account, or edit an existing form previously created to add file upload. Click here to learn how to create a form using Form Builder.

File Upload ๐Ÿ“‚

Once you've named your form and chosen the requirements and permissions, click on Insert Content

Select the File Upload question type.

๐Ÿ’กPlease Note: that you can currently receive the following file types: JPG, PNG, PDF. You will have the ability to add a question or description above the upload button.

File Download โฌ‡๏ธ

To download the file, click on the assigned form from the Appointment Summary card. You'll receive an email notification when the client completes the form.
