De-duplicate roster contact creation via online booking

• Originally, if a patient already exists in the system as a Roster contact and goes on the booking page to book an appointment with another provider, a new contact with the new provider will be created.
• After this change, if the patient goes on the booking page to book an appointment with a new provider using the same email address and name, a new Roster contact will not be created
